Sex discrimination questionnaire (SD74)

A questionnaire prescribed under section 74 of the Sex Discrimination Act 1975 (SDA 1975). Those who believed they had been discriminated against under the SDA 1975 could serve the SD74 on the alleged discriminator to obtain information about the treatment in question or the employer's general treatment of women or men to decide whether to bring an employment tribunal (www.practicallaw.com/9-200-2984) claim.

NOTE The SD74 form pre-dates the Equality Act 2010. If any of the matters complained of occurred after 1 October 2010, employees must use the Discrimination questionnaire (Equality Act 2010) (www.practicallaw.com/0-503-4387) instead (see Practice note, Discrimination in employment: bringing and defending claims (EqA 2010): The questionnaire procedure).
